I'm Irene, a Taiwanese-born, Japanese-licensed pharmacist working in a Japanese drugstore. Intimate itching can be awkward to ask about, so let's start with the two things that matter most: where it itches, and what each tube is for.
Choose by the skin problem, then the texture
For suitable external-skin itching or irritation, Feminina S is a cream-style ointment; Feminina J is the gel option if you prefer a lighter, less sticky feel. UPa is a different formula, aimed at inflamed skin irritated by urine or pads. None of these treats vaginal candidiasis (thrush).
If the itch is inside the vagina, at its opening or around the urethra, or you have unusual discharge, seek medical assessment rather than choosing one of these tubes. Do not use these products if you have candidiasis. Do not use them on mucous membranes or combine them with candidiasis medicines. Knowing the cause matters more than choosing a format.
What about the mist? Kobayashi's exact product page says manufacturing and shipping have ended and sales end when remaining stock runs out. Some brand pages still show it. Treat it as a discontinued product, not a reliably available fourth option.
Three packages to recognize
All three are non-steroid, Class 2 OTC medicines in Japan. “Non-steroid” does not mean suitable for every rash or unrestricted use.

Feminina S ointment: a cream-style choice
Look for フェミニーナ軟膏S, sold in 15 g and 30 g sizes. Despite “ointment” in the name, Kobayashi describes a spreadable, non-sticky cream texture, without menthol or fragrance. It suits someone who wants to apply a cream directly to a small area of appropriate external-skin irritation.
Its listed uses include itch, contact irritation, eczema, insect bites, dermatitis, hives, heat rash, superficial skin erosion and chilblains. Severe weeping or erosion needs advice before use; that indication is not permission to treat badly damaged skin yourself.

Feminina J gel: choose the feel, not a strength ranking
The box says フェミニーナジェル; the formal medicine name is フェミニーナ J, 15 g. The manufacturer describes a smooth, less sticky gel without cooling menthol ingredients. Its listed uses match S, but the active-ingredient formulas differ. Do not interpret the different diphenhydramine amounts as a simple “twice as strong” comparison: one contains a salt form and the other the base.

Feminina UPa: urine- or pad-related inflammation
Look for フェミニーナUPa, 15 g. This cream adds the non-steroid anti-inflammatory ingredient ufenamate and glycyrrhetinic acid. Its Japanese leaflet lists dermatitis, itching, eczema, contact irritation, diaper rash, superficial erosion and heat rash. It relieves an appropriate skin complaint; it does not treat urinary leakage itself or an infection inside the vagina.
What is in each formula?
These amounts are per 100 g of medicine, not the amount to apply. The table lists active ingredients, not every additive; check the full leaflet for allergies.
| Product | Active ingredients per 100 g |
|---|---|
| S ointment | Lidocaine 2.0 g; diphenhydramine hydrochloride 2.0 g; isopropyl methylphenol 0.1 g; tocopherol acetate 0.3 g |
| J gel | Lidocaine 2.0 g; diphenhydramine 1.0 g; isopropyl methylphenol 0.1 g |
| UPa cream | Ufenamate 5.0 g; diphenhydramine 1.0 g; glycyrrhetinic acid 0.3 g; tocopherol acetate 0.5 g; isopropyl methylphenol 0.1 g; lidocaine 1.0 g |
Lidocaine provides local itch relief, diphenhydramine is an antihistamine, and isopropyl methylphenol is an antiseptic ingredient. The presence of an antiseptic does not make these antifungal treatments. More active ingredients do not make UPa a universal upgrade.

How to use, and when to stop
- Check that the problem is suitable for an external skin medicine. Clean the affected skin and surrounding area gently before application.
- Apply a suitable amount several times daily, as directed by the exact package. Use externally only; avoid eyes and mucous membranes. S and J must also not be used around the eyes. For S and J, the leaflet also says not to combine other topical medicines on the same site.
- Wash medicine from your hands before touching your eyes. If medicine gets into an eye, rinse promptly with water or lukewarm water; seek eye care if symptoms are severe.
- Stop and consult if S or J has not helped after 5–6 days, or UPa after about 1–2 weeks. These are no-improvement limits, not instructions to wait despite worsening symptoms.
Stop immediately and consult a doctor, pharmacist or registered medicine seller for rash, redness, increased itch or swelling. UPa additionally lists stinging, a hot sensation and dryness. Bring the box or leaflet so the exact formula can be checked.
Children, pregnancy and other precautions
Before using any of these, ask for advice if you are being treated by a doctor, have had a medicine allergy, or have severe weeping or erosion. The S and J leaflets also require consultation for pregnancy or possible pregnancy and for infants/young children. Kobayashi's current S FAQ specifies consultation for children under seven; children seven and older still need close adult supervision. Do not transfer this FAQ's age detail to another formula.
UPa's leaflet does not state a numerical minimum age or a pregnancy/breastfeeding-specific rule. That absence is not an assurance of suitability: this guide focuses on adult shopping, and I recommend checking with a professional before using it during pregnancy, breastfeeding or for a child, especially for an undiagnosed intimate rash. All three require guardian supervision when used by children.
Keep containers tightly closed in a cool, dry place away from direct sunlight and children's reach. Do not transfer the contents or use an expired product; keep UPa away from fire.
My shelf-side recap
Choose S for a cream-style texture, J for a gel feel, and consider UPa for suitable urine- or pad-related skin inflammation. First-time, unexplained or mucosal symptoms deserve assessment before shopping. You do not need to know the diagnosis yourself: bringing the package name and explaining exactly where it itches is a useful first step.
