Choose standard Melty Cream Lip for its soft, cream-like glide; consider Premium if a richer, more closely adhering moisture layer appeals to you. Both Japanese fragrance-free sticks are untinted, 2.4 g cosmetics. The one-point SPF difference is a poor reason to choose between them.

I’m Irene, a Taiwan-born, Japan-licensed pharmacist working in a Japanese drugstore. My starting point with lip care is whether you want everyday moisture or need help with painful, inflamed or broken skin. Then the texture becomes an easier choice.

The two Japanese fragrance-free sticks
Look for the complete name and the fragrance-free label, not just gold packaging. Standard Melty Cream Lip now also has a gold-toned pack. Other fragrances and tinted Mentholatum products are outside this comparison.

Standard: a soft, creamy everyday option

Rohto describes a smooth stick that seems to melt with lip temperature. Its “Moist Bank” technology is positioned around holding moisture, with squalane and three ceramides highlighted as moisturising ingredients. For someone who wants a soft, untinted stick without added fragrance, that is a straightforward everyday proposition.
Rohto also lists the standard version as an option before lipstick. These descriptions come from the manufacturer; this is not my wear test or a measured ranking of stickiness and moisture duration.
Premium: richer moisture and closer adherence

Premium retains the soft, cream-like approach. Rohto’s main distinction is its “Rich Vaseline” formula: a higher petrolatum content within the Melty Cream Lip series, positioned to cling to the lips and retain moisture.
Consider it if a richer, enveloping moisture layer sounds appealing. The maker’s adherence claims do not establish a fixed number of hours or that the balm stays intact through eating, drinking and wiping. Premium is still a cosmetic, not a stronger treatment medicine.
SPF25 versus SPF26: keep the difference in perspective
Standard is SPF25 / PA+++; Premium is SPF26 / PA+++. That label difference alone does not demonstrate a meaningful real-world protection advantage for Premium. Neither number gives you a guaranteed number of hours outdoors.
Do not treat a moisturised feeling as proof of all-day UV protection. After meals, drinks or wiping, check whether product has been removed and reapply as needed. Rohto’s general lip-care article also recommends cleaning the lips after meals and applying balm before refreshing lipstick. This is practical routine guidance, not a tested reapplication interval for these two sticks.
Fragrance-free still deserves an ingredient check
Both Japanese ingredient lists include petrolatum, squalane, shea butter, ceramides 2, 3 and 6II, honey, royal jelly extract and macadamia seed oil. Fragrance-free describes a fragrance choice; it does not guarantee freedom from allergic reactions. If you know you react to an ingredient, check the current pack or ask Rohto before buying.
If you have specific UV-filter preferences, check the current full ingredient lists before buying; both official lists are linked below. An ingredient’s presence alone does not rank irritation risk for every reader.
Handle the soft stick gently
- Twist up only a little and glide gently over the lips. Avoid extending a long section or pressing hard. Premium’s official warning says its soft formula can break or crumble if extended too far.
- Close the cap securely and keep away from heat and direct sunlight, out of infants’ reach. A sun-heated car is a poor storage spot. Premium’s official precautions explicitly include heat, sunlight and tightly closed storage.
- Do not use either product on wounds, swelling, eczema or other abnormal areas.
- If redness, swelling, itching, irritation or another abnormal reaction appears during use, or after sunlight exposure following use, stop and consult a dermatologist. Continued use may worsen symptoms.
Painful cracks need a different decision
These sticks are for cosmetic moisture care with UV labels. If your lips are broken, repeatedly inflamed or painful, do not simply move up to Premium and keep experimenting. The separate Medical Lip medicine comparison explains another product category; ask a pharmacist or doctor about your symptoms. For other untinted daily-care sticks, see the DHC lip balm comparison.
My shopping shortcut: match the exact fragrance-free name, then choose the moisture feel you want. A one-point SPF difference does not need to add another stick to your basket.
