Allegra FX vs Alesion20 in Japan: Doses, Timing and Driving Warnings

Representative Japanese Allegra FX 28-tablet and Alesion20 24-tablet packages Japan Drugstore Guides

Sneezing and a runny nose on a trip to Japan: should you pick the purple Allegra FX box or the blue Alesion20 box? First check whether either medicine is suitable, then compare morning-and-evening dosing with one bedtime dose. Driving plans matter too.

Both relieve nasal allergy symptoms caused by pollen, house dust and similar allergens: sneezing, a runny nose and a blocked nose. Neither product in this comparison is for anyone under 15 or anyone who has had an allergic reaction to that product or its ingredients. Alesion20 must not be taken by people diagnosed with liver disease. If you are pregnant, might be pregnant, are breastfeeding, receive medical treatment or use other medicines, get advice before choosing.

Irene

I’m Irene, a Taiwan-born, Japan-licensed pharmacist working in a Japanese drugstore. Let’s match the dosing instructions to your travel plans, so a “less drowsy” message on the box does not hide a restriction that matters to you.

Japanese-market products checked on 12 October 2026. Products with the same brand name in other countries may have different ingredients, strengths or directions. Follow the information supplied with your Japanese purchase.

A quick way to narrow the choice

  • Need to drive or operate machinery? Exclude Alesion20. Its label prohibits those activities after taking it. Allegra FX’s Japanese leaflet does not contain the same prohibition, but does list sleepiness as a possible adverse effect. That is not a guarantee of safe driving. Check suitability first, and do not drive if sleepy or dizzy.
  • Prefer one dose before bed? Compare Alesion20, provided its age, liver-disease, interaction and driving restrictions do not rule it out. Bedtime dosing does not cancel the driving warning the next day.
  • Comfortable with morning and evening doses? Compare Allegra FX. It can be taken on an empty stomach; the two daily tablets must not be taken together.
  • Not sure whether this is an allergy or a cold? Ask first. Do not add a cold medicine just because both problems can cause a runny nose. Severe nasal blockage also deserves advice before self-treatment.

The practical differences

What to checkAllegra FXAlesion20
Name on the Japanese boxアレグラFXアレジオン20
Age15 and older15 and older
Active ingredient per tabletFexofenadine hydrochloride 60 mgEpinastine hydrochloride 20 mg
Dose1 tablet twice daily1 tablet once daily
TimingMorning and eveningBefore bed
MealsMay be taken on an empty stomachMay be taken before or after food; keep the bedtime schedule
DrivingNo equivalent prohibition in the leaflet; sleepiness is still possibleDo not drive or operate machinery after taking it
A particularly important exclusionListed interacting medicinesDiagnosed liver disease; driving or machinery operation

These are different second-generation antihistamines. You cannot compare 60 mg with 20 mg to decide which is stronger. A once-daily schedule is not evidence of better symptom relief, and “less drowsy” does not mean side-effect-free.

Allegra FX: one tablet in the morning and one in the evening

Look for FX on the purple package, then check the tablet count. The official page lists 14-, 28- and 56-tablet packs of the same adult formula. The manufacturer recommends starting with a small pack if you have not used it before.

For ages 15 and older: take one tablet twice daily, in the morning and evening. It can be taken on an empty stomach. Swallow with water after removing the tablet from its blister packaging.

  • Each tablet contains 60 mg of fexofenadine hydrochloride: 120 mg across the two daily doses.
  • Do not take both tablets at once or increase the dose because symptoms are worse.
  • If symptoms have not improved after one week, consult a doctor, pharmacist or registered medicine seller. Consult before continuing beyond two weeks, even if it is helping.

The useful travel question is whether you will remember both doses. An irregular breakfast schedule does not require waiting for food, but a missed dose is not a reason to double the next one.

Alesion20: one bedtime dose, with a driving restriction

Find 20 on the blue package. That is the amount of active ingredient per tablet, separate from the number of tablets or days printed on the box. Each white to pale-yellow film-coated tablet contains epinastine hydrochloride 20 mg.

For ages 15 and older: take one tablet once daily before bed, with water or lukewarm water. The manufacturer says food is not required and suggests about 30 minutes before bed. Do not add a morning dose if symptoms return during the day.

  • Do not take it if you have been diagnosed with liver disease.
  • Do not drive or operate machinery after taking it, even if you do not feel noticeably sleepy. Do not invent a safe driving interval based on having slept overnight.
  • First-time users are advised to start with a 6- or 12-tablet pack. A larger pack does not authorize uninterrupted use for all the days it contains.
  • If symptoms have not improved after about one week, stop and seek advice. Consult before continuing beyond two weeks, even if it helps. For both the one-week and two-week checkpoints, count time taking other epinastine-containing medicines too; changing brands does not restart the clock.
  • Ask before switching from a medicine containing epinastine hydrochloride 10 mg. Do not change treatment simply by comparing the numbers.

Do not take these two medicines together. While taking either, do not use other allergy medicines, including medicines for allergic skin conditions or oral rhinitis medicines, or oral medicines containing antihistamines. These can include cold remedies, cough medicines, motion-sickness tablets and sleep medicines. Show a pharmacist the names or package photos of everything you already take, including prescriptions.

Allegra FX also specifically prohibits use with antacid preparations containing aluminium hydroxide and magnesium hydroxide, erythromycin, and apalutamide. The latter two are an antibiotic and a prostate-cancer medicine. Do not stop prescribed treatment to take an OTC allergy tablet, or assume an online spacing rule makes the combination appropriate for you.

Eye drops, nasal sprays and painkillers also need an ingredient check; their dosage form alone does not establish compatibility. If you already have a cold remedy, the Pabron Select comparison can help identify the package before a pharmacist checks the combination. It is not a recommendation to take them together.

Both products say not to drink alcohol before or after taking them. Neither instruction gives a universal number of hours after which drinking becomes safe. Mention planned alcohol use before buying.

When to get advice or stop

Ask before use if you are pregnant or might be pregnant, are an older adult, receive medical treatment, have a history of medicine or other allergies, or have been diagnosed with another allergic condition such as asthma or atopic dermatitis. Long-term conditions, including liver or kidney problems, and prescription treatment need an individual check. Alesion20’s diagnosed-liver-disease prohibition is explicit.

Do not self-select either product while breastfeeding. Both labels require avoiding the medicine or avoiding breastfeeding while using it. Ask a clinician to plan treatment and feeding; do not stop breastfeeding on the basis of this comparison alone.

Stop and seek advice for new symptoms such as a rash, swelling, palpitations or difficulty urinating. Also stop and consult if dry mouth, constipation, diarrhoea or sleepiness persists or worsens. Breathing difficulty, swelling of the throat or lips, or reduced consciousness needs immediate medical help.

Yellow skin or eyes and dark-brown urine can indicate serious liver trouble. Allegra FX also warns about sudden high fever, chills or a sore throat; Alesion20 warns about unusual nosebleeds, bleeding gums or bruising. Stop and obtain immediate medical care for these warning signs rather than waiting a week.

Quick questions

Which one is less likely to make me sleepy?

Both are marketed as less likely to cause drowsiness, but individual responses vary. This comparison cannot promise that either will leave you unaffected. The key practical distinction is Alesion20’s explicit driving prohibition; Allegra FX has no equivalent leaflet prohibition but can still cause sleepiness.

Can I take one for any runny nose?

Check the cause first. Their Japanese OTC indications are for nasal allergy symptoms. Fever, a significant sore throat or uncertainty about allergy versus a cold is a reason to seek advice. They are not general-purpose cold remedies, and their OTC indications do not cover every skin allergy either.

Can a child take half a tablet?

No. The adult Allegra FX and Alesion20 products covered here must not be used below age 15. Allegra FX Junior is a separate product; reducing the adult dose does not create a children’s dose.

Can I take it throughout the pollen season?

Do not decide by the number of boxes you bought. Both require consultation beyond two weeks, and each has instructions for symptoms that have not improved after around one week. Recurrent or worsening symptoms need reassessment.

Before buying, check the full product name, age, other medicines, driving plans and intended duration with the pharmacist. Those details are more useful than asking which box is “strongest.”
